Quick answer
Magellan Healthcare, a Centene company since 2022, manages behavioral health for health plans, employers, state and local governments and federal programs. Its 2026 national handbook sets no single appeal deadline: options after a denial depend on the insurance type and regulations, and the determination letter states them. Claim payment appeals are due within 90 days of payment.
| Situation | Route | Window | Where the rule comes from |
|---|---|---|---|
| Utilization-management denial | Provider dispute, or invoking the member's appeal rights | As stated in the determination letter | Magellan 2026 national handbook |
| Payment disagreement | Claims appeal | Within 90 days of payment, unless rules differ | Magellan 2026 national handbook |
| Claim submission | Timely filing | 60 days from the date of service, or as the contract says | Magellan 2026 national handbook |
| Medicare Advantage plan that delegates to Magellan | Medicare Advantage reconsideration; reopening where criteria are met | 65 days from the notice | Handbook; 42 CFR 422.582 |
| Medicaid program (example: Idaho Behavioral Health Plan) | Appeal to Magellan, then to the state | 60 days from Magellan's decision; 120 days for the state appeal | Idaho Department of Health and Welfare |
Who does Magellan manage behavioral health for?
Magellan Healthcare, formerly Magellan Behavioral Health, became part of Centene on January 4, 2022. Its 2026 handbook says it serves commercial health plans, employers, state and local governments and the federal government, and acts as a delegated vendor for Medicare Advantage plans. Customers change: Blue Cross and Blue Shield of Texas took two products back in 2026.
Magellan's products include employee assistance programs, autism services and behavioral care management. Because Magellan usually works on behalf of a health plan or a state, the plan's appeal rules and state law govern; the determination letter names them. For Blue Cross and Blue Shield of Texas Blue Advantage HMO and MyBlue Health members, the Blue plan itself has administered behavioral health since January 1, 2026.

What are Magellan's appeal deadlines?
Magellan's national handbook defers to the product: the type of insurance and applicable regulations define the options, and the initial determination letter sets out appeal rights. Its fixed numbers are for claims: appeals of payment within 90 days of payment, and claim submission within 60 days of service, unless the contract or law says otherwise.
Medicaid programs Magellan runs follow the federal managed care rules: 60 days to appeal, 30 days for a standard decision and 72 hours for an expedited one. Idaho's Magellan-run Behavioral Health Plan publishes exactly those numbers, plus 120 days to take an upheld decision to the state. Medicare Advantage plans follow CMS reconsideration rules.
Dispute or member appeal: which Magellan option should you choose?
Magellan lets the provider open its own dispute, called an appeal, grievance or dispute depending on the product, or invoke the member's appeal rights, which may need the member's permission. In expedited cases, a clinician who knows the member's condition can act as the authorized representative. State clearly which option you are using.
- Provider dispute: the provider contests the decision in its own name under the contract and product rules.
- Member appeal: the provider acts for the member, with permission where required; this usually carries external review or fair hearing rights.
- Medicare Advantage: Magellan applies an extra reopening option when the reopening criteria are met.
- Payment only: a claims appeal within 90 days of payment, separate from the clinical decision.
How do you get a peer-to-peer with Magellan?
When Magellan cannot authorize the requested level of care, its handbook says it will offer the provider the opportunity and contact details to discuss the decision with a Magellan peer reviewer, and it facilitates peer-to-peer discussions on request. The handbook publishes no window, so ask immediately and have the documentation ready.
The handbook expects providers to be readily available, with documentation, for a reversal discussion after an adverse determination. Use the peer-to-peer prep sheet and record the reviewer's name, credentials and the outcome.
Which clinical criteria does Magellan say it uses?
Magellan's primary tool is Magellan Care Guidelines, which incorporate MCG's behavioral health guidelines and Magellan's own guidelines for specialty outpatient services. It uses the ASAM Criteria for substance use, LOCUS, CALOCUS-CASII and ECSII where a state or customer contract requires them, and CMS coverage determinations for Medicare members.
| Criteria | When Magellan applies it |
|---|---|
| Magellan Care Guidelines (including MCG behavioral health guidelines) | Primary decision support for utilization management |
| Magellan Healthcare Guidelines | Specialty outpatient services |
| The ASAM Criteria | Substance use services |
| LOCUS, CALOCUS-CASII, ECSII | Where a state or customer contract requires them |
| CMS national and local coverage determinations | Medicare members |
| Other customer-required or state criteria | As the contract requires; posted on MagellanProvider.com |
How do you get the criteria and the claim file from Magellan?
Magellan says it makes its care guidelines available to providers free of charge, online where publisher copyrights allow and otherwise on request, and that all level-of-care guidelines are available at your request. The handbook lists an email address for MCG guideline requests. Ask for the claim file in writing in the dispute or appeal.
In Medicaid programs, the plan must provide the case file free of charge and in time for the appeal, and federal parity rules require disclosure of mental health and substance use criteria on request. The criteria request letter drafts both requests.
Where do you submit a Magellan appeal?
Start with the determination letter, which names the route, and Magellan's provider site, MagellanProvider.com, or a contracted vendor's site where Magellan directs you. Medicaid and state programs publish their own contacts. Keep proof of every submission, because the claims appeal clock runs from the payment date.
Which denial codes come up on Magellan behavioral health claims?
Magellan remittances use the standard reason codes. Medical necessity (50) and denial at authorization (39) usually lead to a dispute or member appeal; precertification absent (197) and timely filing (29) to the authorization and submission records; and benefit limits (119) or non-covered charges (96) to the plan's coverage terms.
| Code | What it means (paraphrased) | Usual first step |
|---|---|---|
| CARC 50 | The payer considers the service not medically necessary | Clinical appeal with the record and the criteria applied |
| CARC 39 | Denied when authorization was requested | Appeal the utilization-management decision, not the claim |
| CARC 197 | Required precertification or authorization was absent | Check the authorization record; ask about retrospective review |
| CARC 29 | The filing deadline has passed | Find clearinghouse or portal proof of on-time submission |
| CARC 119 | The benefit maximum for the period has been reached | Check the visit or day limit and whether it raises parity issues |
| CARC 96 | Non-covered charge; a remark code explains why | Check the plan's exclusions; consider a parity question |
| CARC 151 | The records do not support the number or frequency of services | Send documentation tied to each unit, day or hour |
| CARC 185 | The rendering provider is not eligible for the service billed | Check credentialing and supervision billing rules |

A fictional Magellan outpatient therapy dispute
Summit Psychology (fictional) received a Magellan determination limiting outpatient therapy sessions for a commercial member. The letter offered two paths: a provider dispute or invoking the member's appeal rights.
Because the member wanted to keep external review available, the practice invoked the member's appeal rights with the member's signed permission, stated that choice in the first line of the letter, and asked for the Magellan Care Guideline applied. A separate underpaid claim went through Magellan's claims appeal within 90 days of the payment date.

What a ClaimParrot packet does here
- For a Magellan Healthcare denial, an appeal packet drafts the letter for the appeal route this guide describes, and the deadline plan uses the date on your notice.
- Each packet holds an appeal letter draft, an evidence checklist and a deadline plan, checked by a person before delivery: $29, or $99 for level-of-care denials, and the first packet is free (pricing).
- You copy the payer's reason into the form with a de-identified summary; no notices, remittances or records are sent, and no PHI is accepted (Trust & security).
Every draft is reviewed and approved by your team. ClaimParrot does not make medical-necessity determinations or give legal advice.
Frequently asked questions
Is Magellan still an independent company?
No. Centene completed its acquisition of Magellan Health on January 4, 2022. Magellan Healthcare continues to operate under its own name and provider handbook as a Centene subsidiary.
What is the difference between a Magellan provider dispute and a member appeal?
A provider dispute contests the decision in the provider's own name under the contract. A member appeal invokes the member's rights, may need the member's permission, and usually keeps external review or fair hearing rights available. Magellan asks you to say which one you are filing.
Does Magellan use LOCUS?
Only where a state or customer contract requires it. Magellan's handbook names Magellan Care Guidelines as its primary tool, ASAM for substance use, and LOCUS, CALOCUS-CASII and ECSII where required.
What changed for Blue Cross and Blue Shield of Texas members in 2026?
Blue Cross and Blue Shield of Texas said that from January 1, 2026, it, rather than Magellan, administers behavioral health benefits for Blue Advantage HMO and MyBlue Health, honoring existing Magellan authorizations for up to 180 days.
