Quick answer
Evernorth Behavioral Health, formerly Cigna Behavioral Health, manages mental health and substance use benefits for Cigna Healthcare plans. Coding and fee disputes can go through reconsideration; medical-necessity and level-of-care denials go to appeal within 180 calendar days, filed by the patient or the provider. Evernorth uses MCG Behavioral Health Guidelines for mental health and the ASAM Criteria for substance use.
| Dispute | Route | Filing window | Decision time |
|---|---|---|---|
| Coding edit or fee-schedule dispute | Reconsideration on Provider.Evernorth.com (an option, not a required step) | Finalized claims only | Routed to the adjustment team or the appeals team |
| Medical necessity, level of care or length of stay | Appeal by the patient or the provider | 180 calendar days from the denial | Most within 60 calendar days; notice within 75 days |
| Precertification denied, services not yet provided | Customer (patient) appeal | Per the plan; Evernorth's member form says 180 days | Expedited if urgent and the patient is still in that level of care |
| Second-level appeal, where offered | Appeal | 60 days from the first-level decision letter | Per plan and state |
| After internal appeals | External review for the patient (some appeals) or arbitration for contract disputes | Arbitration within one year of the final internal decision | External reviewer or arbitrator decides |
Who manages behavioral health for Cigna Healthcare plans?
Evernorth Behavioral Health, a subsidiary of The Cigna Group and formerly Cigna Behavioral Health, is the behavioral network for customers with Cigna Healthcare plans. It also sells behavioral services to employers without Cigna medical coverage. Its September 2026 administrative guidelines are the behavioral provider manual and form part of the provider contract.
Cigna sold its Medicare Advantage, Part D and related businesses to Health Care Service Corporation (HCSC) on March 19, 2025, so former Cigna Medicare members now appeal through HCSC's plans. Cigna's current provider appeals page no longer covers Medicare.
What are Cigna's appeal deadlines and decision times?
Cigna's provider page allows 180 calendar days from the initial payment or denial notice to appeal, with review completed within 60 days and notice within 75 business days. Evernorth's behavioral guidelines add that clinical coverage appeals are due within 180 days and second-level appeals within 60 days of the first decision.
Processes vary with state mandates and contract terms, and Evernorth's state addenda may not apply to self-funded plans. Arbitration is available only after internal appeals are exhausted and must be requested within one year of the final internal decision. For some medical-necessity appeals, Cigna can offer the patient external review by an independent review organization, which needs the patient's approval.
Who can appeal which Cigna behavioral health denial?
Cigna's own table assigns appeal rights by scenario. Precertification denials for services not yet provided are customer appeals. Reimbursement disputes and denials for precertification not obtained are provider appeals. Medical-necessity, level-of-care, length-of-stay and experimental denials can be appealed by either the patient or the provider.
| Scenario | Who appeals |
|---|---|
| Precertification denied, services not yet provided | Customer (patient) |
| Precertification not obtained, services denied | Provider |
| Reimbursement: bundling, modifiers and similar | Provider |
| Inpatient facility level of care, length of stay, delayed treatment day | Either |
| Medical necessity | Either |
| Experimental or investigational | Either |
| Benefit denial or maximum reimbursable amount | Customer (patient) |
Evernorth's September 2026 guidelines say appeal requests may not be submitted using automation software or systems: each appeal must be submitted by a person, including when a billing service files it. Software can prepare the packet, but a person on your team must submit it.
How does peer-to-peer review work at Evernorth?
Evernorth treats peer-to-peer review as the step before a formal clinical appeal: a board-certified psychiatrist or doctoral-level psychologist discusses the case, and afterward a physician not previously involved can review the decision. Evernorth publishes no national request window and encourages sending clinical information before the conversation.
State addenda can set timing. Massachusetts, for example, requires a peer reconsideration within one business day of the request. Reach the care manager on the case or the provider services line in the guidelines, and keep the peer-to-peer prep sheet to one page.
Which clinical criteria does Evernorth say it uses?
Evernorth uses MCG Behavioral Health Guidelines for mental health level-of-care reviews unless law or contract requires otherwise. It uses LOCUS and CALOCUS-CASII for commercial plans in states that require them, and the ASAM Criteria for substance use: the fourth edition for ages 18 and older since November 2024, the third for ages 17 and under.
Cigna stopped using its own mental health medical necessity criteria on November 27, 2020; archived versions remain online for older cases. Each adverse-determination letter should name the specific guideline used and the clinical rationale.
| Criteria | Applies to | Notes |
|---|---|---|
| MCG Behavioral Health Guidelines | Mental health level of care (default) | Published through an MCG portal whose terms bar use with AI or machine-learning systems |
| LOCUS | Commercial plans, ages 19 and older, where state law requires | Evernorth's pages name California and New York; one page adds Colorado |
| CALOCUS-CASII | Commercial plans, ages 6 to 18, in the same states | Free family and provider guides are posted |
| The ASAM Criteria, 4th edition | Substance use, ages 18 and older (since November 2024) | Full text behind the Provider.Evernorth.com login |
| The ASAM Criteria, 3rd edition | Substance use, ages 17 and under | Same |
| Cigna and Evernorth coverage policies | Autism (ABA), TMS and similar services | Coverage Policies page |
How do you get the criteria and the claim file from Cigna?
Each Evernorth adverse-determination letter must name the guideline used and the clinical rationale, and coverage guidelines are posted on Provider.Evernorth.com. Evernorth publishes no provider claim-file procedure, so ask in writing, in the appeal, for the criteria applied and all documents relevant to the claim, citing the federal disclosure rules.
Plans must disclose mental health and substance use medical-necessity criteria on request to participants and contracting providers, and employer plans must give claimants free copies of relevant documents. Name the criteria in your letter and quote only what the plan sent you; do not paste licensed guideline text into the appeal or into AI tools. The criteria request letter drafts the request.
Where do you submit a Cigna behavioral health appeal?
Behavioral health providers use Provider.Evernorth.com, where the reconsideration tool routes each finalized claim to the adjustment team or the national appeals team. Written behavioral appeals go to Evernorth's Central Appeals Unit with the Behavioral Appeals Cover Sheet, which asks for the issue ID from the denial letter. Follow the letter's instructions.
Corrected claims cannot go through the online reconsideration tool, and an open draft must be submitted within five calendar days. Out-of-network providers may need an appointment-of-representative form signed by the patient.
Which denial codes come up on Cigna behavioral health claims?
Cigna's remittances use the standard claim adjustment reason codes. The codes that decide the route include medical necessity (50), which either party can appeal, precertification absent (197), which is a provider appeal, benefit limits (119) and non-covered charges (96), which are often customer appeals, and coding codes such as 4 and 97.
| Code | What it means (paraphrased) | Usual first step |
|---|---|---|
| CARC 50 | The payer considers the service not medically necessary | Clinical appeal with the record and the criteria applied |
| CARC 197 | Required precertification or authorization was absent | Check the authorization record; ask about retrospective review |
| CARC 198 | Services exceeded the precertification or authorization | Request an extension or appeal the concurrent-review decision |
| CARC 39 | Denied when authorization was requested | Appeal the utilization-management decision, not the claim |
| CARC 119 | The benefit maximum for the period has been reached | Check the visit or day limit and whether it raises parity issues |
| CARC 96 | Non-covered charge; a remark code explains why | Check the plan's exclusions; consider a parity question |
| CARC 97 | Paid as part of another service already adjudicated | Check bundling policy, such as psychotherapy add-ons |
| CARC 4 | The modifier does not fit the procedure code, or a required one is missing | Check telehealth, ABA or provider-level modifiers and send a corrected claim |

A fictional Evernorth residential denial
Lakeshore Residential (fictional) treated a 17-year-old Cigna member for a substance use disorder. Evernorth denied days 12 to 20 after a peer-to-peer, citing the ASAM Criteria. The program noted that for a member aged 17, Evernorth applies the third edition, while its own assessment used the fourth.
The appeal, filed by a staff member through Provider.Evernorth.com rather than any automated tool, answered the third-edition dimensions the reviewer cited and attached the complete facility record, as Evernorth asks for length-of-stay denials. It requested the guideline named in the letter and calendared the 180-day window from the denial date.

What a ClaimParrot packet does here
- For a Cigna / Evernorth Behavioral Health denial, an appeal packet drafts the letter for the appeal route this guide describes, and the deadline plan uses the date on your notice.
- Each packet holds an appeal letter draft, an evidence checklist and a deadline plan, checked by a person before delivery: $29, or $99 for level-of-care denials, and the first packet is free (pricing).
- You copy the payer's reason into the form with a de-identified summary; no notices, remittances or records are sent, and no PHI is accepted (Trust & security).
Every draft is reviewed and approved by your team. ClaimParrot does not make medical-necessity determinations or give legal advice.
Frequently asked questions
Is Evernorth the same company as Cigna?
Evernorth is a subsidiary of The Cigna Group. Evernorth Behavioral Health is the behavioral network for Cigna Healthcare plans and was formerly called Cigna Behavioral Health. Cigna Healthcare is the health plan; the denial letter names who decided.
Does Cigna require reconsideration before a medical-necessity appeal?
No. Cigna presents reconsideration as an option for coding edits and fee disputes on finalized claims. Medical-necessity and level-of-care denials go to appeal, which either the patient or the provider can file.
Can billing software submit Evernorth appeals automatically?
Not under Evernorth's September 2026 guidelines, which say appeals may not be submitted using automation software or systems and that each appeal must be submitted by a person, including by billing services. Tools can help prepare the packet; a person must submit it.
Where do former Cigna Medicare Advantage members appeal now?
Cigna sold its Medicare Advantage and related businesses to Health Care Service Corporation on March 19, 2025. Follow the plan name and instructions on the current denial notice; Medicare Advantage appeals then follow CMS rules, starting with a reconsideration within 65 days.
