Anthem Blue Cross and Blue Shield behavioral health appeals

How behavioral-health appeals work for Anthem Blue Cross and Blue Shield members: who decides, clinical appeals, payment dispute windows by state, peer-to-peer and BlueCard.

Quick answer

Anthem Blue Cross and Blue Shield plans are Elevance Health companies, each an independent licensee of the Blue Cross Blue Shield Association. For Anthem commercial behavioral health, Carelon Behavioral Health generally handles utilization management while Anthem handles claims and payment disputes. Clinical appeals generally have 180 days; payment dispute windows vary by state. Check the card and the notice.

Who handles behavioral health for Anthem plans?

Two companies share the work. For Anthem commercial and Medicare Advantage members in the states Carelon documents, Carelon Behavioral Health provides utilization management on Anthem's behalf: authorizations, peer reviews and clinical denials. Anthem keeps claims and claim payment disputes under its own provider manual. Some employer accounts route behavioral health differently.

Anthem Blue Cross and Blue Shield operates in Colorado, Connecticut, Georgia, Indiana, Kentucky, Maine, Missouri, Nevada, New Hampshire, Ohio, Virginia and Wisconsin, with some county exclusions, plus part of New York. Behavioral networks have been moving into Carelon's network state by state, most recently Virginia commercial in 2026. Read the ID card and the denial letter to see who decided.

02

What are Anthem's appeal deadlines?

Anthem's 2026 commercial manuals give clinical appeals 180 calendar days in most states and 120 in Connecticut, with standard post-service decisions within 60 days. Claim payment disputes run in two steps, reconsideration then appeal, and the windows vary by state, from 120 days in Connecticut to two years in Indiana.

The claim payment dispute process does not cover clinical decisions such as a denied authorization; those go through clinical appeals. Each step can be filed once, and both must be finished before contractual dispute resolution or arbitration. Late requests without extenuating circumstances are ineligible, and the provider then cannot bill Anthem or the member. Maine, New Hampshire and Wisconsin follow the Ohio pattern; Nevada and Colorado use different structures.

03

Payment dispute or clinical appeal: which Anthem route applies?

Use the claim payment dispute for payment and processing problems, including claims denied for no authorization when one was obtained. Use a clinical appeal for medical-necessity decisions. Expedited and pre-service clinical appeals are always handled as member appeals, and a provider filing for the member needs a signed Designation of Representation form.

  • Payment dispute: wrong payment amount, bundling (CARC 97), or a CARC 197 denial when you hold the authorization number.
  • Clinical appeal: level-of-care or medical-necessity denial (CARC 50), through Availity's Authorizations & Referrals application where available, verbally or in writing.
  • Member appeal: expedited and pre-service appeals; a provider who asks for expedited review is treated as the member's designated representative.
  • Carelon letter: when Carelon issued the behavioral health denial, follow the appeal instructions in that letter.
04

How do you request a peer-to-peer with Anthem?

Anthem's manuals let a treating practitioner, or their designee, request a peer-to-peer when an adverse determination will be or has been made; hospital representatives, employers and vendors cannot. No numeric window is published. Anthem announced that from September 1, 2026, behavioral health peer-to-peer conversations can be scheduled in Availity Essentials.

The Availity scheduling covers commercial and Medicare Advantage members and replaces phone and email requests. Georgia's commercial notice also says the conversation must happen before all applicable appeals are completed. Keep the call focused with the peer-to-peer prep sheet.

05

Which clinical criteria does Anthem say it uses?

Anthem says it uses its own clinical guidelines and is licensed to use MCG guidelines, including MCG's Behavioral Health Care Guidelines, which are proprietary and not published online. Where Carelon manages behavioral health, Carelon's criteria order, including ASAM and InterQual, may apply instead. Ask which set and edition the reviewer used.

Anthem's clinical guidelines page says hard copies of MCG content can be requested through the number on the member's ID card. Denial notices should name the criteria used; if one does not, say so in the appeal. See the Carelon guide for Carelon's criteria order.

06

How do you get Anthem's criteria and the claim file?

Anthem says providers can request a free copy of its utilization-management criteria from its medical management department, and hard copies of MCG and clinical guidelines through the number on the member's ID card. Anthem's manuals describe no provider claim-file request, so ask for the file in writing in the appeal.

Plans covering mental health and substance use benefits must disclose medical-necessity criteria on request to participants and contracting providers, and employer plans must give claimants free copies of documents relevant to the claim. The criteria request letter asks for both.

07

Where do you submit an Anthem appeal?

Availity Essentials is Anthem's preferred channel: the Claims Status application for payment reconsiderations and appeals, and the Authorizations & Referrals application for clinical appeals where available. Reconsiderations can also be made by phone or in writing, and written appeals go to the address in your state's provider manual.

Include a position statement, the provider's name, NPI or TIN and contact details, the member's name and Anthem ID, claim numbers and dates of service, and supporting documents. Answer requests for more information within seven days.

08

What changes when the patient has another Blue plan?

Under BlueCard, you file the claim with your local Blue company, but the member's home plan is responsible for precertification and utilization-management appeals. Anthem's manual says that for a medical-necessity claim denial without home-plan instructions, the local Anthem plan is the contact, and home plans may require their own representation form.

The Blue Cross Blue Shield system is made up of 33 independent, locally operated companies, each licensed for its own area. Never borrow another Blue company's deadline, form or criteria; the prefix on the ID card routes the claim to the right company.

09

Which denial codes come up on Anthem behavioral health claims?

Anthem claims use the standard reason codes. Medical necessity (50) leads to a clinical appeal; payment codes such as bundling (97) and modifiers (4) lead to a payment dispute; authorization codes (197, 198) depend on whether you hold the authorization; timely filing (29) and provider eligibility (170, 185) need records from your own systems.

Fictional example

A fictional Anthem payment dispute that was really a clinical appeal

Maple Counseling Group (fictional) in Ohio saw IOP claims for an Anthem commercial member denied as not medically necessary after Carelon's concurrent review. The biller opened a claim payment reconsideration in Availity, then noticed that Anthem's manual excludes clinical decisions from that process.

The team withdrew the plan, followed the appeal instructions in Carelon's letter instead, and filed a clinical appeal well within 180 days with the clinician's letter and group notes. A separate underpayment on two other dates went through Anthem's payment reconsideration, inside the 365-day Ohio window.

Names, dates, payers and amounts are invented for illustration.

Where ClaimParrot fits

What a ClaimParrot packet does here

  • For a Anthem Blue Cross and Blue Shield denial, an appeal packet drafts the letter for the appeal route this guide describes, and the deadline plan uses the date on your notice.
  • Each packet holds an appeal letter draft, an evidence checklist and a deadline plan, checked by a person before delivery: $29, or $99 for level-of-care denials, and the first packet is free (pricing).
  • You copy the payer's reason into the form with a de-identified summary; no notices, remittances or records are sent, and no PHI is accepted (Trust & security).

Every draft is reviewed and approved by your team. ClaimParrot does not make medical-necessity determinations or give legal advice.

Common questions

Frequently asked questions

Is Anthem the same company as Blue Cross Blue Shield?

Anthem Blue Cross and Blue Shield plans are Elevance Health companies and independent licensees of the Blue Cross Blue Shield Association. Other Blue plans are separate companies with their own rules, even though they share the brand.

Does Carelon or Anthem decide an Anthem member's behavioral health appeal?

It depends on the decision and the state. Carelon generally decides utilization-management questions for Anthem commercial behavioral health, while Anthem handles claim payment disputes. Follow the appeal instructions in the letter that issued the denial.

How long do we have to dispute an Anthem payment in Ohio?

Anthem's 2026 Ohio commercial manual allows 365 days from the explanation of payment for a payment reconsideration and 90 days from that decision for the payment appeal, unless state law or your agreement requires otherwise.

Do we need a Designation of Representation for every Anthem appeal?

No. Anthem's manuals say no designation is required when a provider appeals on its own behalf. It is needed when the provider appeals for the member, which includes expedited and pre-service clinical appeals.
